Please keep in mind that Facebook may take 5 days to two weeks to re enable your account after you have submited the information as well.

FAcebook just takes a while to look over thr form and make thier decison.  How long ago did you submit the form?

Also, the page was set up for me by someone else. At some point I must have changed the password because that person no longer has access to the page

The fan page is conncted to the disabled account so only way is to get back in the disabled account.

Well messenger you can still use as its hopefully not disabled from your phone.   The Meta Suite will be disabled until I can get you back into your account.
